Needs exam: A 32 y/o with a sensation of a lump in the throat with negative assessment by ENT/Pulmonary: think about esophageal disease (spasm, reflux, diverticulum, etc), enlarged thiroid, congenital malformation such as thyroglossal cyst, vascular anomaly, etc.
Psychosomatic causes can also be considered.

Chest pain?: The mood swings and anxiety are very commonly associated with adrenal and thyroid problems while chest pain may accompany the anxiety, it needs to be evaluated separately to rule out other causes including those related to the heart, lungs or stomach.

Less likely: The thyroid would be an unusual spot for breast cancer to spread. More common sites would be bone, liver, lung and brain. But with cancer, anything is possible. If there is a nodule on the thyroid the best thing to do would be an ultrasound guided biopsy if it looks concerning.
