Get checked : She should be checked for TB and MAI - risk of TB is very high in South Africa. With her CD4 count under 100, she could have cryptosporidiosis, microsporidiosis, CMV reactivation. Check stool studies for these conditions even stool AFB. My main concern would be ruling out TB - when did she start antiretrovirals ? She could have immune reconstitution, when an existent problem emerges .
