Yes: To have pain there must be some sort of pressure stretching the ear drum. (in or out) To have drainage you either have a hole in the eardrum with pus or debris draining or an inflamed canal draining. Fever may not be present if your body walls off the process. However, you can have an otitis with fluid that does not require antibiotics.These can be monitored without treatment.
