Earache-wait?: Hard question. Pain is your body's way of saying that ain't right. Generally i would say two days, you should be seen. Barring obvious signs of serious illness and a contraindication to using decongestants, afrin nasal spray, otc antihistamines like Claritin or zyrtec and valsalva maneuvers (popping ear like you coming down a mountain) may do the trick. Don't forget teeth can cause ear pain.

Any hearing loss: The first question i ask a patient with ear ache (otalgia) is if there is any hearing loss associated with pain. If there is no hearing loss and particularly if the pain is in front of the ear, i would be thinking tmd. This could be joint disease, muscle spasm or referred pain to the side of the face (ear) from dental disease as the other reviewer suggested.
