Sesamoid pain- xray (clean), pain subsided and then returned worse 2nd xray clean so diagnosed as sesmaoiditis; could there be a stress fracture?

Yes: An MRI can be helpful in seeing a sesamoid stress fracture, stress reaction (bone edema, no fracture), bursitis, or flexor tendinitis in that area. Sesamoid problems are notoriously slow to heal.
