Painful swelling: get it checked by doctor. some times the pain may just be because of chostochondritis. If there is a swelling in question please get it looked at. Given that it is painful- it is likely to be a inflammatory condition but will need examination.

Are they related?: If the pain is in the protrusion, the 2 symptoms are probably related. There can be multiple causes for this, but most commonly in a healthy young man it is due to a strain or inflammation of the rib joint, called costochondritis. Other causes include vitamin D deficiencies, breast gland growth, and RARELY-tumors in the rib bone.

Pectus excavatum: Some people develop a degree of deformity to chest at ribs and sternum. You should have a doctor look at it.

Good question: There are a number of rib abnormalities that are congenital. If you have not had it checked out yet, please go see your primary care provider. It may be worth getting an x-ray of the ribs to see what that protrusion really is and if it could be causing that pain.
