Usually people need antibiotics if they have signs of infection like pain and fevers. some times they may be required of there is chronic low grade temperature with pain and congestion. Rarely : The vast majority of sinus infections are viral and therefore do not respond to antibiotics. The usual course for a otherwise healthy non-smoker is 7-14 days for a sinus cold. Sudafed and an antihistamine (like Claritin or Zyrtec) can be used along with Flonase for symptom relief.
