25yr old male held urine from 6-7pm but he had used it @4:50pm now he is experiencing pain in area under testicles (not anus) what's cause?

Not clear: but any pain in/around testicles could be a torsion which is a medical emergency. He should be examined as soon as possible to avoid losing the testicle if this is the problem.
