Concerning...: You don't give a past med history or age, in order to ans this question the most import things to know is do you have DM or a history of vascular disease. This could be a hematoma on the underside of the toe which in that area can be very painful since the trapped blood causes pressure buildup. More concerning is possib of gangrene, espec if you have DM. Need more info. Rec pod, wound or HT cons.
