Need more detail: Unlikely TB, would have shown up on the xray. Could be inflammation of the bronchioles/airway. Don't think it's anything dangerous like cancer or bacterial infection. Depends on what your blood tests and most importantly your clinical exam showed. Could be heavy metal exposure, allergic reaction, asthma, anxiety, stress, sinus infection.

More testing: In my opinion,These symptoms call for collaboration among internal medicine specialist, pulmonologist and cardiologist. A thorough history, complete physical examination and appropriate tests to also include comprehensive blood tests, echocardiogram,, sputum studies, careful nose and throat exam,checks for anemia,alpha trypsin deficiency,etc. Searches for causes of true clubbing by each specialist
