Recheck it.: I don't think that reading is accurate. The systolic and diastolic numbers are too similar. Would recheck it but I don't think a systolic pressure of 154 warrants an ER visit.

No ...: ... unless you are in the second half of pregnancy - high blood pressure in pregnancy can be serious, at this threshold possibly requiring monitoring / treatment. High blood pressure is diagnosed over a period of time, if you have consistent readings of this level it is recommended you take action. See your Dr to confirm & initiate investigations. In the meantime reduce salt + increase exercise.

Depends: If you are having severe headache with nausea , vomiting, neck pain , light is annoying your eyes then please go to ED otherwise book an appointment with your GP. If repeat Bp is high then you would need investigations and medications. Reducing salt intake, exercise , try to loose weight if overweight would also help to lower blood pressure .
