About 1/200: We don't see it as much now as we did many years ago due to increased recognition and treatment of strep. However, the risk of complications like Rheumatic fever is still present, primarily in children. The cited risk per untreated strep infection is 1/200 and can be reduced/eliminated if treatment is initiated within 10 days of first symptoms.
