Further work up need: The most important thing w/ PSA is the gradient (How high has it been before) or the absolute value above 4. There are patient with PSA's between 1 and 4 who DO have disease but most don't There are other tests such as the free PSA ratio in blood and the PC3 test in urine. A PSA alone gives us a 60% chance of accuracy and a rectal exam adds another 25 points of accuracy; so you need both.
