I had surgery on my right arm 3 years ago because of cut open veins and today i hit a wall with my fist the knuckle on the middle finger got swollen, ?

Get x-rays: Hitting your fist against a wall is a common way to sustain a fracture of your metatarsal (hand bone). Since it is swollen, it is a good idea to get an x-ray to make sure nothing is broken. The previous surgery 3 years ago likely has no effect on the current injury.
