White/black: Gray scale differences on CT scans are simply representations of the X-ray attenuation(how much of the X-ray beam is absorbed) of various structures. It is analogous to density. By convention, if a structure is less dense, it is darker, if more dense, whiter. For example, air is black and bone is white on CT. It has nothing to do with osteomyelitis, which shows up as destruction of bone.
