Many possibilities: Rashes that remain localized to specific areas are often reactions to soaps/lotions or other material that was in contact with that area.Specific cause is hard to confirm without an exam and review of possible exposures.

Rule out measels: 4 year old little gentleman with rash developing on top of ear lobes and spreading along hairline to cheeks. This history is textbook for the presentation of measles. Is your son vaccinated? Fever? Several rashes affect children including viruses, bacteria, allergic reactions. Most are self limited but measles is one not to miss. I would bring him for a full exam and blood test for measles.
