The most important: medical issue during the horrific time of explosion or mass shooting is the effectiveness of the emergency response system. Many cities and communities in the United States have updated and refined their emergency systems to deal with these new threats: faster, more coordinated response; capacity for more individuals responding; training for in-field care. Death. The best: defense against these acts is to be elsewhere. Failing that, find a safe area and stay in it for long periods of time. Failing that, have emergency first aid available in multiple locations. Same with food and "camping" supplies, since infrastructure is likely to be interrupted.
