The most important: medical issue during the horrific time of explosion or mass shooting is the effectiveness of the emergency response system. Many cities and communities in the United States have updated and refined their emergency systems to deal with these new threats: faster, more coordinated response; capacity for more individuals responding; training for in-field care. Emergency response seems to be excellent.

Here are some...: 1. with common sense, learn more on how to minimize & even avoid unnecessary injury on site; 2. manage to evacuate from the site without further provoking the perpetrator(s); 3. seek counseling for the event and life reality after the event; 4. take a follow-up counseling in weeks to months after the event.

Death. The best: defense against these acts is to be elsewhere. Failing that, find a safe area and stay in it for long periods of time. Failing that, have emergency first aid available in multiple locations. Same with food and "camping" supplies, since infrastructure is likely to be interrupted. I grieve for the state of the world when this kind of question shows up on Healthtap...

Multiple: Eliminate the threat; treat the wounded; bury the dead; never forget: to remember those we love, and protect them from those craven souls who would inflict mayhem with reckless abandon.

Medical Issues: I'm not sure I understand your question because in events of mass injuries all medical issues are present with lifesaving emergency intervention first.

Posttraumatic stress: Humans react uniquely to terrorist, explosive, mass shooting incidents. Some people have a thick skin. Others develop hypervigilance panic attacks anxiety depression. The only way to sensitively specifically know is to give internet based actuarial assessment tests. An unstructured psychiatric physical exam has a hit rate or success of 49% compared with 97% using tests. Knowing allows managing.
