Shoulder pain: I think you are letting your anxiety get the best of you. No injury or trauma. Sounds perhaps positional. Just relax and do passive shoulder exercises and see how you feel heat the shoulder and then your passive movements

Hypnagogic.: This sounds like what we would call a "hypnagogic hallucination". This happens juuuuust as our brain is slowing down enough to put us asleep, and we experience a sudden awakening jolt of the body. This can be associated with what seems like a very brief and violent dream. We term them "hypnapompic" if they happen upon awakening in the morning, but more common at night.

Muscle Jerking when: falling asleep is normal and is referred to as myoclonus. It usually means there's a lot of tension in your muscles, and jerking is the sudden release as you relax to sleep. You have anxiety already. Are you taking meds for the rhinitis? One side effect of antihistamines is muscle rigidity. Try some yoga before bedtime. Alcohol is a bad idea in this case.
