Start with internist: While it might be ideal to see a psychiatrist, due to lack of access I advise you see a general internist or family doctor, as they are trained to deal with just about every condition. Your problems could be related to illnesses such as thyroid disorders or infections like Lyme disease, so it's reasonable to see someone who can assess everything, If issues are psychiatric they can start treatment.

Yes, that's the: profession to start with. Consider Healthtap's Concierge--a service ideally suited for just this kind of Tx. Check with your insurance as more of them are paying for this services (phone and/or telehealth in general; not necessarily Healthtap, specifically). Sleep and food changes need to be addressed STAT as they (initially, sleep more) influence just about everything else.
