Yes: It's not clear as to why the patient is taking Coumadin (warfarin). The therapeutic range of Coumadin (warfarin) starts at an INR of 2.0 as in atrial fibrillation patients. She is not therapeutic yet, so she should continue with the lovenox. If Coumadin (warfarin) was prescribed following valve surgery, her target INR is usually 2.5 and therefore should take lovenox until INR is at that level. Talk to INR clinic.
