Yes: Lovenox (enoxaparin) keeps you anticoagulated while waiting for your warfarin to reach a therapeutic level. If you don't take this as a bridge, you may form a bigger clot.

Usually: Unless you were on Heparin or one of the other lmwh's (like arixtra), yes it is recommended to be anticoagulated when starting warfarin. This is because there is a brief increase in coagulation tendencies by release of the vit-k dependent clotting factors w/start of warfarin. Once depleted (few days), the Lovenox (enoxaparin) can be stopped.
