Possible yes: If the dose is suboptimal or the person is deficient in coagulation factors not affected by Lovenox (enoxaparin) you can still get a blood cloth.

Certainly: Lovanox is a powerful anticoagulant, but it can be under-dosed, particularly in obese patients, or patients with a predilection to clot. Your doctor can order a xa level (pronounced "ten-a") to check the effectiveness of lovanox.
