That is a specific: question for the Neurosurgeon, especially one familiar with your particular case. If you have exhausted all other options, as you have listed, then surgery would have to be the next viable step, given the severity of symptoms that you cannot live with. And that is an important distinction: you cannot live with this pain. Surgical options are almost always the last.
