Maybe not Gut: could be ur ribs or rather the small joint spaces in between the ribs called costochondritis which is better treated with ibuprofen or NSAIDS in general. Key is it if it hurts when u press on area or take deep breaths. With IBS, constipation can be an issue, and if that is in ur case, then working on ur Bowel movements is key with fiber, hydration, stool softeners or laxatives.

More info needed: Your symptom of left rib to back pain is nonspecific and more description is needed in order to make a diagnosis. A possibility is a pinched intercostal muscle or nerve which runs between two ribs and causes sharp,shooting rib cage pain worsened with a deep breath or other chest motions. The IBS doesn't appear related. Your MD,after a history and physical, will order appropriate tests and xrays.
