Many possibilities : There are several possibilities and without more information, I would be guessing. She still could be constipated, have esophagitis or something else. I would see her doctor. But consider keeping a diary of her symptoms in the meantime.

Sounds like : she is suffering from chronic constipation causing the "accidents" as in fecal soiling. Need to discuss with your pediatrician - make sure she does not have urinary tract infection also. Increase fluids, fibers, fruits ,water and take laxatives every day to avoid fecal impaction, blood in the stools, recurrent abdominal pain.Good luck!
