Damaged molar: A damaged tooth even in the absence of pain should be treated as it may cause more serious problems in the future. If you are a dental phobic, try seeking recommendations from other people who have similar fears for a referral to a dentist who is sensitive to your similar fears. Getting help early will increase your chances of saving your teeth.

Some advice: I you don't go to the dentist you may well end up in the hospital. Untreated decayed teeth usually result in infections that can spread & cause serious complications. You probably have other teeth that have decay and may also have gum disease. Dentistry can be performed under IV sedation so you will be "asleep" for everything. Only you can make yourself go. Perhaps speak to a therapist about it.

Floss, Floss, Floss!: If you won't address the issue, then at least take care of what remains. In that I am certain you have been toothbrushing since you we're a child, maybe trying something different like flossing daily!
