Orthopedic surgeon: Best bet is the Orthopedic Surgeon or a sports medicine MD, who will likely refer you to the Surgeon if the injury requires it. Important at your age to receive evaluation, diagnosis and treatment ASAP to prevent long term problems. Stand your ground and lobby for the earliest appt.

Your PCP : Is a good place to start as PCPs see shoulder injuries all the time. Otherwise being seen by an orthopedic surgeon or physiatrist would be fine too.
