Most likely because: In my experience the most likely cause is an opiate use disorder not recognized. Indeed, I would assume so given the doses, duration of care, your age, and troubles you are having in reducing the dose. Patients who have opiate use disorders benefit from medication assisted treatment whether they have pain or not. Check out DrRotchford.com and elsewhere for information about same.
