Real problems with k: Is there a family history of polycystic kidney disease? The right kidney seems to be very enlarged Average size at 28 wks gestation is only 34 mm + or - 4 mm. I do not know if the hospital has a kidney maven to discuss this with you but you should also both speak to a genetic counselor after this pregnancy for recurrence risk assessment. Cystic kidneys: Sounds like either ARPKD ( Autosomal recessive polycystic kidney disease) or renal dysplasia due to obstructive uropathy. Both these conditions are compatible with life depending upon fetal lung maturity. Believe it or not-- you need normal kidneys for lungsto develop in utero. In any case my best advice to you would be to consult with a Pediatric Nephrologist, before the birth of the baby.

Evaluation at birth: The OB doctor can follow the infant before birth. A neonatologist needs to evaluate the kidneys after birth sonogram of the kidneys afterbirth is much more accurate. If the neonatologist is concerned a referral can be made to a pediatric urologist for further evaluation. At this point is a wait and see period. Don't assume that surgery is necessary.
