Stable: You must be in a stable position before this surgery, or there will not be any reference position for positioning and no stability. Also, after healing the displaced disks might not reposition properly and the surgery will have been for naught. Get stable first. Often this requires splint therapy (24/7) for 6-8 months or as long as it takes to recapture and reposition the disks. 2nd opinion.

Average relapse 3%: Open TMJ disc repositioning surgery can be used for prevention of relapse with double jaw surgery in patients with TMJ internal disc displacement. However, more thorough evidenced-based studies are required to understand the response of the TMJ disc stability after mandibular advancement surgery.

Complicated: Very complex question. For best results work with a team that includes an oral surgeon VERY experienced in orthognathic surgery in TMJ patients, and an orthodontist VERY skilled in jaw surgery and TMJ patients.
