See surgeon: Goldenhar syndrome is a rare congenital defect characterized by incomplete development of the ear, nose, soft palate, lip, and mandible. You should consult surgeon to make sure symptoms are not related to change in mandible since original surgery. Immediate treatment could involve intermittently icing and applying heat to the TMJ joint. Some nonsteroidal antiinflammatory mediction may help.

More info needed: Do you wear an appliance called a splint (or a night guard) to reduce the stress of clenching and grinding? If not, it would be wise to have a dentist make you a custom-fitted guard (not a flexible mouthguard, and not anything over-the-counter, as these can make things worse), which will not only protect your joints and dentition, but should lessen the pain associated with TMJ issues.
