Pain, nausea,: Vomiting, increase in size, failure to pass stool or gas, hernia gets hard, any of these symptoms can mean emergency surgery.

You'll know: The definition of "worse" is different among patients, but regarding hernias this generally refers to pain and discomfort, and enlarging size. There are some things that could be emergently worse such as a blockage of, or strangulation of the intestine. Best to have a consultation with a general surgeon to find out about your specific case.
