Follow-up: I feel that such an enlarged lymph node must have follow-up which includes a careful examination by an otolaryngologist or a head and neck surgeon and a decision to make regarding repeat examination,blood work or a decision to biopsy it. It may well be benign,but I prefer to have a specialist involved in the evaluation.

2.8cm or 2.8mm?: Size makes a huge difference in evaluating lymph nodes with imaging. That being said, size is not the only story and abnormal lymph nodes don't always have to be enlarged. Further imaging with PET/CT can be obtained but that's usually saved for known cancer pts. Confirm the measurement and follow up with your PCP / ENT regardless.
