Chronic paronychia: This sounds like a paronychia, which is an infection of the skin around the borders of the nails. Acutely, these can be caused by bacteria and need drainage, antibiotics and warm soaks. Infections that are recurrent or resistant to ths treatment are often caused by yeast or fungus and will need a different medication. Surgery is sometimes needed with severe infections.

See doctor: You should see your doctor as this may need antibiotics or possibly surgery to drain pus.

See a hand surgeon: In the early stages, an acute paronychia can be effectively treated with hot saline (salt) water soaks. Antibiotics and surgery is rarely needed. In the late stages when pus has accumulated under the parionychium, incision and drainage is necessary- followed by warm saline soaks. Antibiotics may also be prescribed in this case. Chronic paronychia may require anti-fungal medication and surgery.
