If: Custom made (not OTC) bite guard should provide relief. If it does, your occlusion should be evaluated, perhaps by an Orthodontist. If it does not, the next step would be to see an Endodontist.

Try the guard: If your are clenching and/or grinding your teeth, then the guard will help. If you've cracked the tooth, the guard will not provide relief.

See DDS: Bruxism can cause tooth pain, but is usually not limited to one tooth. There are many variables which your dentist will address, is there currently a restoration in the tooth? How deep is the restoration if present? When was the restoration placed? Did the tooth have a root canal? Did the tooth have a crown? Once all scenarios are addressed, the dentist will be better to ascertain.
