Evaluation: Establishing care with primary care physician who will guide you through evaluation and refer you to a specialist if appropriate would be a reasonable thing to do. You say that you have a history of "bad knee" , and it seems that it has been getting worse. You need a clinical evaluation, likely followed by Xrays, and maybe MRI. Depending on results, the treatment options can be discussed.
