Needs to see a doc: He needs to get over his fear. Explain in a nice way he may need an antibiotic and further evaluation and this is the job of a doctor. Ensure him you will be with him and he will be okay.

See a doctor: He at least need to see a doctor. His symptoms sounds like a problem with circulation including a possible blood clot.
