Mono: If his tonsils are sticking out without any other symptoms, like breathing difficulty, vomiting, inability to drink or eat, yes go to the ER. Mono tends to make all lymph nodes swollen and take sometime to get better

Probably OK: He should go to the ER only if the nodes cause difficulty breathing or swallowing. Hopefully, the nodes will slowly go away soon!.

Mono: yes: especially if it is tender or if he has problems with breathing----it is normal during mono to have enlarged tonsils liver and spleen--the swelling can be decreased with a coarse of steroids-- and you can always get a secondary bacterial infection when they are enlarged Go see the doctor-----if the doctor deems that an antibiotic is needed-do not go on amoxicillin-it is associated witha rash

Maybe/not: If he is sleeping & drinking fluids okay & able to smile & play some there is no reason to go.Many kids have quite generous tonsils/nodes even without mono & if they are not interfering with sleep of fluid intake the ER would do nothing. If the kid can't/won't drink, can't sleep & hasn't peed in 12 hours he is getting dry & needs to go in.
