R/o infection: Take your son to a pediatric orthopedist and have him look for possible infection of the spinal column.

MRI needed...?: Back pain in a 3 year old is concerning, especially if he is having night sweats. He may require more imaging like an mri, or more imaging depending on what area the ct covered. Another thing to consider if it is low back pain is whether he is having any urinary symptoms? Kidney infections can cause low back pain.
