Diet : Commonest cause of this is lousy diet. A good diet is expensive and many people cannot afford it. I have seen this many times, especially in adolescents. Commonest cause is vitamin b1 deficiency caused by taking sugar in all its different commercial forms. It is a sad comment on U.S. Diet.
