Brushing wrong: The most common reason i see is that people are ineffective at oral hygiene. Most don't clean the most important sites--in between the teeth. Also, when they brush, they brush the chewing surface of their teeth instead of removing the plaque from the gum line--the junction of the teeth and gums. Have a good dental check-up and have a hygienist demonstrate proper techniques.
