Warfarin is traditional drug of choice but requires strict nutrition (vitamin k) & regular monitoring. Novel oral anticoagulants are available w/o regard to food or regular testing.

It varies: With thrombophlebitis (deep venous thrombosis), the treatment is anticoagulation. This involves an infusion of Heparin or injections of a heparinoid like Lovenox (enoxaparin) initially for about 4 to 7 days, with long term treatment with warfarin (coumadin). The resolution of clots can take months and may never be complete, depending on the the extent of the dvt. Venous ultrasounds will monitor the progress.
