What is treatment for repeated vomiting diagnosed with closed duodenum for man 47 years old having diabetes?

Complex issue: Need to find out why a closed duodenum-scarred, ulcer, tumor mass of stomach, duodenum, pancreas, other. Diabetes can slow stomach emptying, but not close duodenum. May need some manner of bypass procedure if truly obstructed. Definitely needs to see doc with some experience in this.
