I Can Only Guide: Severe depression is a medical illness and therefore must be evaluated by a physician. One's primary care physician (pcp) can take a comprehensive history, perform a physical examination, and order any necessary tests or studies to diagnose depression. Once diagnosed, the pcp may treat the depression or refer one to a psychiatrist or licensed therapist for treatment. Seek help now, don't wait.
