Have you tried thera: Could be tendonitis which may need anti-inflammatory meds and physical therapy. Have an MRI to determine what cause is.

Nerve entrapment: Nerve entrapment is another possible cause of pain on the top of the foot.

It would help if you: Provided more of a history. Why did you go to the doctor in the first place? Was there a history of trauma? "top of foot pain" is the only clue you provided. Does the pain tingle or shoot? Is it always present? Knowing where anatomically on the foot would help as well. Anterior tarsal tunnel could cause top of the foot pain and provide a negative mri, and persist if untreated.
