It depends: Assuming you are referring to ankylosing spondylitis, the first step is a thorough history and physical by a qualified specialist. If warranted, xrays may be taken to look at the sacroiliac joints and/or lumbar spine. Mri of the si joints/spine may also be useful early in the disease.
