Bad breath: I hope he must be flossing everyday if not let him start flossing especially at night time. One of the cause could be acid reflux or gastric related. You may see your physician if it is the cause . One may try peroxyl mouthwash , dilute and use , try for atleast a week however ultimately your dentist is the best judge.

Medical issues?: If he was thoroughly evaluated by the dentist for periodontal (gum) disease and if it was determined that he did not have this problem, and if he has had his teeth thoroughly cleaned recently and if he is a meticulous dental maintainer and if he does not have odor producing foods in his diet, then the next step would be a medical evaluation. Time to see you physician.

Medical issue?: Assuming a thorough exam was performed, the problem might be of a gastric nature. Also, pay attention when happens. Is it noticeable after certain foods or is it during a certain time of day. A visit to your physician should be considered to rule out or confirm any possible underlying problems.

Stomach: Many times bad breath is not caused by anything in your mouth but from your stomach since the dentist found no problems i would -see another dentist and if there are no problems, see a physician.
