I would: The less extra stuff in your mouth for the doctor to work around the better. Also, an electrosurgical instrument is sometimes used to cauterize the area where the tonsils are removed. While this is a bit away from your tongue piercing, these instruments do not interact well with other metal objects...Best to be safe and remove it.

Yes.: Any time you undergo surgery under general anesthesia you will be asked if there is anything removable in your mouth. If there is (and tongue rings can be removed) they will insist you remove it.
