Consensus helps...: Given that four different doctors gave the same diagnosis (assuming they were unbiased and didn't know about one another's diagnosis), i would tend to believe it. The proof is if you get better from the treatment they recommended. If a standard treatment (or two) does not work, then calling into question the original diagnosis is quite reasonable.

Yes: Folliculitis doesn't require any testing. In fact, most diseases can be diagnosed by taking a careful history and doing a physical examination. Visual evaluation of the skin is part of the physical examination. Althought it is possible that a few doctors maight make the same incorrect diagnosis, there is a good chance that they made the right diagnosis since they all came to the same conclusion.

Usually: If there is doubt, and the lesions have pus, a culture can be done. Otherwise, a biopsy would be in order. But folliculitis should not be difficult to diagnose.
