In some cases, yes: Some people can be highly successful in the outer world, but miserable inside. The disturbance they have may be more pronounced in intimate relationships than in external achieving. I have known many high functioning people with character disorders such as narcissistic and borderline. Some have recognized their need for help, and have done the work required to recover. Ongoing process, tho'.
